Changing Your Home Care Provider is Easy
3 EASY STEPS TO CHANGE PROVIDERS.
- Contact Better Living Homecare. Learn how you can receive more hours of care and continue to receive care from your existing carer. Our experienced Case Managers will listen to your individual needs and preferences.
- Notify your existing provider. Let them know that you wish to have your package managed by Better Living Homecare and mutually agree with them the date on which they are to cease managing the package.
- Contact My Aged Care. Request a “Referral Code” and provide consent to My Aged Care for your information to be transferred to Better Living Homecare. My Aged Care can be contacted on 1800 200 422 or through their client portal.
BETTER LIVING HOMECARE WILL TAKE STEPS TO ENSURE MOVING YOUR HOME CARE PACKAGE IS EASY
- Meet with you and work in partnership in preparing your individual care plan.
- Contact your existing Home Care Package provider and confirm the date they are ceasing to manage the package.
- Contact My Aged Care and provide them with your “Referral code”.
- If you choose to continue receiving care from your existing carer, we will contact the home care agency who currently provides your carers (this is often different to the provider who manages your package) and arrange for them to continue to supply your known carers. This is your choice and it should be noted that Better Living Homecare does not charge a surcharge for using carers from different agencies.
- Advise Department of Human Services within 28 days of Better Living Homecare commencing to manage your Home Care Package.
- We will keep you informed and will let you know when Better Living Homecare have received your unspent home care amounts from your previous provider.

STEPS YOUR EXISTING SERVICE PROVIDER WILL BE REQUIRED TO UNDERTAKE
- Mutually agree with you the date on which they are to cease managing your package.
- Advise Department of Human Services within 31 days of them ceasing to manage your Home Care Package.
- Notify you in writing of your unspent home care amount. This must be advised within 56 days of them ceasing to manage your Home Care Package.
- Arrange the payment of any unspent home care amount to Better Living Homecare within 70 days of them ceasing to manage your Home Care Package.
